"Is the licensing risk actually real?"

Yes — and it's enforced.

U.S. copyright law sets statutory damages of $750 to $30,000 per song for unlicensed commercial use. It isn't theoretical: a Tampa bar was fined $30,000 and closed after its music license lapsed; a restaurant was ordered to pay $30,450 — plus legal fees — over four unlicensed songs. Rights organizations find businesses through their own ads and social posts, then send investigators who log every track played.

"But I pay for Spotify Premium…"

Personal streaming accounts are licensed for personal use only. Spotify's own terms prohibit playing it in a business — and there is no "Spotify for Business." Roughly 8 in 10 business owners don't know this. Paying for Premium doesn't change it.
